Classification of periodontal osseous defects (POD) in molars

Defect 0 (Horizontal): Horizontal bone loss without intraosseous component and intact furcation

Defect 1: any interproximal bone defect with an intact furcation

Defect 2: any interproximal bone defect with furcation lesion grade II. The fornix of the furcation is located coronal to the interproximal bone levels. This feature is easily identifiable in lower molars. On the contrary, the identification is more difficult due to the palatal root, thus requiring a meticulous clinical examination of the furcation zone.

Defect 3: grade II or III furcation lesion, no matter how small, in which the fornix of the furcation is located apical to the interproximal bone levels. Interproximal bone loss can range from unnoticeable to 20% of the root length. The are no interproximal intraosseous defects. This defect evolves to defect 4 and later, to 5.

Defect 4: grade III furcation lesion with interproximal bone loss 21% to 40%

Defect 5: grade III furcation lesion with interproximal bone loss >40%

Classification of periodontal osseous defects (POD) in non-molar teeth

Defect H (horizontal): horizontal bone loss without intraosseous component. Bone loss ranges from 10% to 70%. Bone loss exceeding 70% associates with intraosseous component.

Defect 1: Any vertical interproximal defect, with no interproximal bone loss on the other side of the tooth.

Defect 2: Any vertical interproximal defect, with bone loss in the coronal third of the root on the other side.

Defect 3: Any vertical interproximal defect, with bone loss reaching half the root length on the other side.

Defect 4: Interproximal bone loss reaching half the root length, with a circumferential intraosseous component in the middle third of the root.

Defect 5: Interproximal bone loss in the middle third of the root, with a circumferential component reaching the apical third of the root.
